Fixed 2.1.265 regression with CLAUDE_CODE_USE_GATEWAY environment variable
Fixed a regression in 2.1.265 where the CLAUDE_CODE_USE_GATEWAY environment variable was incorrectly forcing Cloud-gateway sign-in even when used with API keys or custom auth headers, causing request failures. The variable is now ignored when used alone, restoring previous behavior without requiring configuration changes.
